The Chemical Dependency Case Manager- Phoenix House, provides outreach and case management under clinical supervision in the outpatient and residential setting. The Chemical Dependency Case Manager maintains a professional, collegial and positive relationship with staff and clients and would need to possess excellent customer service skills, be energetic, confident, dependable, and self-motivated; be a good communicator and listener and with proficient skills with Microsoft Office. Having a health care or mental health background is a plus and be able to work with low income, multi ethnic patient population and demonstrate the ability to relate respectively and effectively to people of diverse racial, educational, and socio-economic backgrounds. You will work with multiple local businesses & partnerships that will benefit our residents, screen potential residents prior to entering the program, assessing the individual’s degree of drug dependency. As part of case management, the case manager meets with residents at least once a week to review wants, needs, program progress, reviews records, and coordinates appointments with health professionals, as needed. Case managers must be able to establish solid boundaries while working with the residents and their families.
