Social Worker/Behavioral Health Consultant

The Social Worker/Behavioral Health Consultant gives priority to the most pressing problem and is a persistent, flexible, organized, adaptable, collaborative strong advocate that thrives as part of a team in a fast paced setting.  The Behavioral Health Consultant, as part of the primary care treatment team, identifies, triages, and manages patients with physical and behavioral health problems within the primary care setting.  The Behavioral Health Consultant develops behavioral health change plans with patients and provides psycho-education and brief therapy focused on improving the patient’s overall health and quality of life. Requires a Master’s or Doctorate degree in social work, marriage and family therapy, psychology, behavioral health or related field, unrestricted Marriage and Family Therapist, Mental Health Counselor or Clinical Social Work license, computer skills, excellent customer service skills, and a desire to work with a low-income, diverse population dealing with behavioral and physical health issues.  Dual licensure as a Chemical Dependency Professional is a plus.
