Gerardo Melgar, MD – Medical Director
Dr. Melgar received a Bachelor of Science degree in Zoology, and a Doctor of Medicine degree from the University of the Philippines. He is board certified in Internal Medicine and received his training at the Cleveland Clinic in Ohio. Dr. Melgar began his community health medicine career in 1996 while working at a Central Washington community health center, eventually becoming Medical Director. He joined the Family Health Center in 2006 as Medical Director, and continues an active outpatient and inpatient practice in internal medicine. He works primarily out of the Longview Medical Clinic.
Anthony Reyes, MD – Associate Medical Director
Dr. Anthony Lirio Reyes received a Bachelor of Science degree in Biology from the University of the Philippines in Manila, and then attended the University of Santo Tomas, Faculty of Medicine and Surgery in the Philippines where he received his Doctor of Medicine degree. He completed his Family Medicine Program residency at the McLaren Regional Medical Center, in Flint Michigan. Dr. Reyes gained experience in the field of medicine while working at Cheery Street Health Services, a community health center in Grand Rapids, Michigan and is certified by the American Board of Family Medicine.

Reinhild Ayoub, MD – Castle Rock Clinic
Dr. Ayoub graduated from the University of Heidelberg Medical School, after studying in France, Germany, and Harvard Medical School. She completed her pediatric residency at New England Medical Center, Tufts University in Boston, MA. Dr. Ayoub has over 30 years of experience in general pediatric medicine and has served the Castle Rock community for many years. She is board certified by the American Board of Pediatrics.
Tresha Carroll, DO – Longview Medical Clinic
Dr. Carroll received a Bachelor of Science degree in Biology and a minor in Fine Arts from the University of Portland in Oregon. She then attended Western University of Health Sciences in Pomona, California earning a Doctor of Osteopathic Medicine degree and completed an Internal Medicine Internship at the Arizona GME Consortium, Tucson General Hospital in Tucson, Arizona. Dr. Carroll completed her Family Medicine Residency PGY 2 at the Arizona GME Consortium, Tucson General Hospital and her Family Medicine Residency PGY3 at Eastmoreland General Hospital in Portland Oregon. She is board certified by the American College of Osteopathic Family Physicians.
Gail Conway, DO – Longview Medical Clinic
Dr. Conway received a Bachelor of Arts in Psychology from Rutgers University in New Jersey, a Masters of Computer Science, and a degree in Biology from the University of Denver in Colorado. She attended Oklahoma State University for her Doctor of Osteopathic Medicine. Dr. Conway has research experience from working at the University of Colorado Health Sciences Center, she was a Family Medicine Resident at the University of Oklahoma, and has been a Primary Care Provider at the Family Health Center since 2002.
Kelsey Fried, ARNP – Woodland Clinic
Kelsey Fried received a Bachelor of Nursing from Gustavus Adolphus College. She then went on to attend Oregon Health and Science University where she earned both a Master of Nursing and Doctor of Nursing Practice. Kelsey is board certified by the American Academy of Nurse Practitioners.

Mary Knutson, ARNP – Longview Medical Clinic
Mary Knutson earned a Bachelor of Science in Nursing degree from Montana State University and a Master of Science in Nursing degree from the University of Washington. Mary has been a practicing Women’s Health Nurse Practitioner specialist since 1975. She was employed by the Washington State Department of Health as a clinical consultant to the statewide family planning program, retiring from the agency in 2014. She is a Women’s Health Care Nurse Practitioner certified by the Nation Certification Corporation and now concentrates solely on direct care for women. In addition, Mary worked for Family Health Center in the 1980’s as a Family Planning Nurse Practitioner. She believes that being involved in women’s health from both a public health perspective and a clinical practice perspective has been an incredibly rewarding experience.

Connie Liu, ARNP – Kelso Clinic
Connie Liu received her undergraduate degree from the University of Washington and her Master of Science in Nursing from Seattle University. Connie performed clinical rotations at Seattle University in family practice, women’s health and rural community health. She is board certified as a Family Nurse Practitioner by the American Academy of Nurse Practitioners. Connie has volunteered her time providing health education and health screenings in underserved communities in Seattle and internationally.
Kaelie Murray, ARNP – Kelso Clinic
Kaelie Murray attended Mesa Community College and Northern Arizona University, earning an Associate Degree in Applied Science, Bachelor Degree in Nursing and a Master Degree in Nursing. She has gained experience by working in several areas of acute care including a Level 2 nursery, pediatrics, postpartum care, bariatrics, medical-surgical, progressive care , outpatient IV therapy and critical care. In addition, Kaelie completed her nurse practitioner clinical rotation in an underserved, rural, Central Arizona community. She is board certified by the American Nurses Credentialing Center and belongs to Sigma Theta Tau International. Kaelie has volunteered her time to provide support for new mothers and breastfeeding initiatives.

Natalie Paul, ARNP – Woodland Clinic
Natalie Paul received a Bachelor of Arts in Liberal Arts degree from the New College of Florida and attended Vanderbilt University, earning a Master of Science in Nursing degree. She completed a two year research fellowship in functional neuroimaging and Duke University before deciding to pursue a career as a Nurse Practitioner. She has also gained experience as a research nurse in oncology and health care informatics. Natalie has been published on three occasions and has a strong interest in evidence-based practice. She is board certified by the American Academy of Nurse Practitioners, belongs to the American Association of Nurse Practitioners and has volunteered her time with the Launch Pad Homeless Youth Shelter and the Shade Tree Homeless Flu Vaccine Outreach Program.

Claudia Sloan, MD – Longview Medical Clinic
Claudia Carlota Sloan is a Medical Surgeon with a wide breadth of education and experience. Dr. Sloan attended the Universidad Nacional de Colombia, the Sackler Faculty of Medicine and the Tel Aviv University in Israel. She completed her residency at the St. Vincent Family Medicine Residency Program and received postgraduate colposcopy training from the American Society for Colposcopy and Cervical Pathology (ASCCP). She has gained experience working in several settings including nursing homes, acute care, community health centers, medical clinics and in veterans health services. Dr. Sloan is a member of the ASCCP and has volunteered with the Northshore Community Church Medical Missions Team, serving in both Nepal and Nicaragua.

Kathleen Wirtz, CNM, WHNP-BC – Woodland Clinic
Kathleen Wirtz is a Certified Nurse Midwife and Women’s Health Nurse Practitioner. She received a Bachelor of Science in Nursing degree from Linfield College and a Master of Science in Nursing from San Diego State University. Kathleen has acquired great experience in the fields of midwifery and women’s health by attending and assisting with in-hospital and out-of-hospital births, and as a Birth Assistant for the Best Start Birth Center in California. She is board certified by the American Midwifery Certification Board and the National Certification Corporation. Kathleen is a returning resident of the local area and takes pride in giving back to the community.

Christine “Chris” Larsen received a Registered Nurse degree from Tacoma General Hospital, a Bachelor of Science in Nursing degree from Pacific Lutheran University, and a Master of Nursing degree from the University of Washington. Chris has been a prescribing ARNP since the mid- 90’s and has over thirty years of mental health nursing experience, with the majority being in community mental health. Jeanne Fischer-McKee, PMHNP – Woodland, Kelso and North Beach Clinics
Jeanne Fischer-McKee is a Psychiatric Mental Health Nurse Practitioner, BC and Acudetox Specialist. She earned an Associate Degree in Nursing from Clark County Community College and then went on to earn both a Bachelor Degree in Nursing and a Master Degree in Nursing from Washington State University. Jeanne has gained experience working in the primary care setting; home health and hospice; and providing mental health counseling. She is board certified by the American Nurses Credentialing Center and is a member of the American Psychiatric Nurses Association and National Acupuncture Detoxification Association.
David Meyers, DDS – Dental Director
Dr. Meyers received a Bachelors of Science degree in Psychology from Arizona State University and a Doctorate of Dental Surgery from the University of Washington. Dr. Meyers joined the Family Health Center in 2012 and has worked in private practice for over 18 years. He is a member of the Lower Columbia Dental Society, the Washington State Dental Association, and the American Dental Association. Dr. Meyers is an active volunteer for the Boy Scouts of America and has helped all five of his sons achieve the rank of Eagle Scout.
Eric Gurrad, DDS – Associate Dental Director
Dr. Gurrad earned a Bachelor of Arts degree in Psychology from the University of Washington, and a Doctorate of Dental Surgery from the University of the Pacific Arthur A. Dugoni School of Dentistry. He is a member of the Lower Columbia Dental Society, the Washington State Dental Association, and the American Dental Association.
Jai Lee, DDS
Dr. Lee attended the Loma Linda University School of Dentistry in California. Dr. Lee has experience in both the community health setting and private practice and sincerely understands the need for providing preventive and restorative oral health care to low income and uninsured populations.
Cuong Luu, DDS
Dr. Luu began working at the Family Health Center after attending the Meharry Medical College School of Dentistry. Meharry chooses students based on their ability to complete the curriculum and their willingness to provide care for underserved populations, such as those served at the Family Health Center. Dr. Luu is member of the Washington State Dental Association and the American Dental Association.
Kristin Nordahl, DMD
Dr. Nordahl earned her Doctorate of Dental Medicine from the Oregon Health State University School of Dentistry. She has practiced at the Family Health Center since 2009. Dr. Nordahl is a member of the American Dental Association and the Academy of General Dentistry.
Esther Su-Young, DDS
Dr. Su received a Doctorate in Dental Surgery from Loma Linda University School of Dentistry in California. She has been with the Family Health Center Longview Dental Clinic since 2000. Dr. Su is a member of the American Dental Association.
Justin Tan, DDS
Dr. Tan attended the Loma Linda University School of Dentistry where he received a General Dentistry and Doctor of Dental Surgery degree. He has worked as a dental assistant and provided dental services during mission trips to Fiji, Peru and Ethiopia. In addition, Dr. Tan has volunteered his time at several clinics providing dental care for the underserved.
Tatiana Trott, DDS
Dr. Trott received her Doctorate in Dental Surgery from Riga Medical Institute in Lativa in 1986, and from the Pacific Dental School in in 2009. She has over 20 years experience in the field of general dentistry and is certified by the Western Regional Examining Board. Dr. Trott volunteered her time between 1995 and 2000 to provide dental care to orphans in Lativa.

Dr. Herman attended the Creighton University School of Dentistry earning her Doctorate in Dental Surgery. She has been practicing general dentistry since 2005 and is a member of the Lower Columbia Dental Society, the Washington State Dental Association, and the American Dental Association.

Dr. Ziegler earned a Bachelor of Science degree in Biology from Pacific Lutheran University and a Doctor of Dental Surgery from the University of Washington School of Dentistry. Formerly, Dr. Ziegler owned and operated Michael H. Ziegler, DDS, PS and Advanced Dental Services, LLC in Longview. He is a member of the Lower Columbia Dental Society, the Washington State Dental Association, the American Dental Association and the American Academy of Dental Sleep Medicine. Dr. Ziegler is also a member of the Rotary Club of Longview.
